BadgerCare Plus

CommunityConnect HealthPlan has been available to BadgerCare Plus members in Kenosha, Milwaukee, Ozaukee, Racine, Washington and Waukesha counties since September 2010. We are now the new health plan choice for BadgerCare Plus members living in Brown, Calumet, Dodge (zip codes 53916, 53048 and 53050 only), Fond du Lac, Manitowoc, Outagamie, Sheboygan, Walworth and Winnebago counties. Besides covering services for medical care, mental health and dental, CommunityConnect has many programs to help you manage your health issues.

Enrollment

You have a few ways to sign up for BadgerCare Plus:

• Apply online at the BadgerCare Access website
• Call us: 1-800-362-3002
• Mail the form you filled out after you got it from the website: dhs.wisconsin.gov/forms/F1/F10182.pdf
• Go to your local county or tribal agency for an interview

You must choose one HMO for all family members. If you do not do this, the Department of Health Services will choose a plan for you. You may change plans within three months and then you must stay with that plan for the next nine months.
